Follow Up Appointment (approx. 30 min): $235.00 / $200.00
Extended Follow Up/Psychotherapy/Family session (if available): $350.00 / $300.00
IDFPR Evaluation for Professional Licensure: Physician $1600, All other health care professions (RN, PharmD, LPN, LCSW, etc) $1200

Please note that if you are using insurance but have not yet met your plan’s deductible, you may be responsible for paying up to a similar amount as listed above. It is strongly recommended you familiarize yourself with your individual plan’s benefits (deductible, copay, coinsurance, etc). For consistency and parity, our fee schedule is intended to be comparable to current in-network insurance reimbursement rates.

The following services rendered by Modern Mental Health providers and staff outside of scheduled appointments are not covered by health insurance and will be charged directly to the patient at the time of service.

– Completion of work releases, disability/insurance forms, detailed letters, etc. – $75.00
– Early or unscheduled prescription refills, including switching pharmacies – $25.00
– Report writing for legal, employment, licensure, etc. – $400.00 and up depending on detail and time required

Same Day Cancellation/No-Show Fee: Appointments that are cancelled on the same day of the appointment, or are not attended as scheduled (i.e., no-show), will be charged the cancellation fee (as noted in the fee schedule) directly to the patient that must be paid before another appointment will be scheduled. As a courtesy, this fee will be waived one time only.

– $200.00 – to be paid before further appointments or treatments provided
